How the All on 4 Treatment Works

How the All on 4 Treatment Works

The two front implants (anterior) are usually placed vertically, and the two at the back (posterior) are placed at an angle to avoid nerves and vital anatomical structures and to utilise existing bone. An immediate loaded arch of teeth is placed 2 hours after the implant procedure and followed later by a permanent prosthesis.

Note: The angle at which the exterior implants are placed is very important

  1. It allows maximum utilisation of the bone
  2. Avoid any vital structures or nerves
  3. Eliminate the need for bone grafts.
  4. It helps with load distribution.

All on 4 Dental Implants Procedure – Step by Step

When it comes to a same-day implant procedure, don’t let the name fool you; there are a lot of steps. I will explore these steps in detail.

Step 1

You will receive a consultation and examination to ensure you are suitable for the procedure. Whether 3D imaging, x-ray, or even digital smile design

Step 2

Surgery Day (Same-Day Teeth)

Duration: 1-2 hours per implant

Damaged Teeth Extracted if needed, Placement of 4 implants, then the placement of a fixed bridge to protect the implants.

Step 3

Healing Phase & Final Teeth

Duration: 6-9 months

Healing period from 6-9 months for the implant to go through (Osseointegration). The Placement of permanent prostheses.

Risks, Disadvantages & Honest Considerations

When it comes to the All on 4 procedure, there are significant health risks. There are precautions to be taken to avoid some of them.

Possible Risks

This occurs when the implant doesn’t undergo osseointegration (doesn’t fuse properly with your bone).

If your jawbone is not strong enough or gets weakened for any health reason. Then, the implant won’t stabilise, leading to failure.

It is considered periodontal disease, but for implants. It occurs when the tissue around the implant becomes inflamed or when the bone around the implant starts deteriorating due to bacteria.

If the implant is placed too close to the nerve, it can cause tingling, or a “pins-and-needles” feeling in the tongue, lips, chin, or gums.

An infection that could be caused by the implant perforating the sinus cavity and could lead to serious complications.

The misplacement of the implant or the wrong fitting implant, or in case the implant shifted, this can cause a misaligned bite, which can cause discomfort or excessive pressure on the implant, leading to TMJ, speech and aesthetic issues as well as a broken prosthetic.

How Risks Are Minimised

You can avoid a lot of these risks if you get the procedure in a certified and renowned place, as well as an experienced surgeon to execute the surgery.

Here are some things to avoid when getting this procedure:

Avoid clinics that push procedures with no risk mentioned (Hard Sell)

Avoid clinics with no certification or credentials listed

Avoid clinics that ignore your concerns and questions.

Check the clinic reviews and the guarantees

Follow an oral hygiene routine daily to avoid failure because of a lack of care

Avoid smoking, eating hard/ chewy foods (a soft food diet is necessary).

Pay attention to consistent pain, wobbly implants or signs of infection.

Plan for the worst and have a return trip ready in case of complications.

Dental Implant Candidate Criteria

Non-Smokers

Smoking reduces blood flow, increases bone loss, and interferes with osseointegration, significantly lowering implant success rates.

Good Gum Health (Tissue Health)

Periodontal disease leads to inflammation, infection, and jawbone loss, compromising implant stability and soft tissue health.

Good Overall Health

Uncontrolled diabetes, autoimmune disorders, osteoporosis, or heart disease may negatively affect healing after implant placement.

Good Bone Density

Bone density above 850 HU (D1 or D2) is essential for implant integration and long-term success.

Socket Condition

Missing buccal plates, residual cysts, or postoperative maxillary cysts can compromise implant placement and stability.

Pros and Cons of Same-Day Dental Implants

There are a lot of advantages and disadvantages when it comes to Same-Day dental implants, and we will elaborate more:

The Pros of Same-Day Implants

  • Better face aesthetics
  • Less surgeries
  • Reduced healing time by half
  • Preserving bone at the extraction site
  • Best utilisation of a bone-implant surface area

The Cons of Same-Day Implants

  • It is not suitable for everyone
  • It could be challenging to stabilise the implant
  • Risk of partial alveolar bone resorption due to traumatic damage during extraction
  • Possible gap between the implant surface and socket wall (Jumping Distance risk)
  • Difficulty achieving complete closure of the implant site

Risk and complications of an Immediate Teeth Loading Procedure

The success rate of same-day implants is around 97.4% according to recent research. However, like other procedures, there are risks and complications that can happen, such as:

  • Failure of the implant to osseointegrate. (heal properly and fuse with the bone)
  • Injury due to excessive implant depth
  • Loss of hard and soft tissue
  • Implant malposition
  • Restorative complications
  • Infections such as sinus, gum, and bone infections.

  1. If you suffer from “persistent or worsening pain, swelling and redness, bleeding or pus, bad taste or breath, bad taste or breath, fever, chills, loose implant”, consult a doctor immediately.

    2.Same Day Implants Aftercare

To heal properly, you need to follow a strict soft-food diet, avoid strenuous exercise, and keep up with your follow-up visits.

Soft Food Diet

During your healing period, you should follow a soft-food diet and avoid crunchy, chewy, or hard foods. You should consume foods such as yoghurt, soups, mashed vegetables, eggs, and smoothies.

Pain & Recovery

It is expected that the first week will be painful; you may experience swelling and discomfort. However, it should improve over time.

Avoid Strenuous Exercise

As with any surgery, exercise is not allowed for a couple of days post-procedure because it can raise blood pressure, increase swelling, increase the risk of discomfort and pain, and interfere with clot formation.

Follow-up Visits

After healing, you need to return to the clinic to receive your permanent crowns. If you used the temporary crowns as permanent teeth, they would break and could compromise the implant's success rate.

Long-Term Maintenance

Implants are like your natural teeth; you need to brush and floss them regularly, and take care of them as you would your natural teeth, or you risk affecting their longevity and shortening their lifespan.
