Who Dental Implants Suit, and Who They Do Not

Implants are not automatically the right answer for every gap. The purpose of the scan and the medical questionnaire is to establish which of the three groups below you fall into before you book flights.

You are likely to be a straightforward candidate if

  • You have one or more missing teeth, or teeth that are failing and cannot be restored
  • Your jaw has finished growing, which usually means you are at least in your late teens or early twenties. There is no upper age limit: general health matters far more than the year on your passport
  • Your gums are healthy, or any gum disease has been treated and is stable
  • You have enough bone height and width at the implant site, or enough to allow grafting
  • You are willing to keep up daily cleaning around the implants and attend reviews once you are home

You will need something dealt with first

  • Active gum disease. Placing an implant into inflamed tissue invites early failure. Periodontal treatment comes first, and the implant plan follows.
  • Thin or resorbed bone. This does not rule you out. It changes the plan to grafting, a sinus lift, or a full-arch design that uses the bone you still have.
  • Uncontrolled diabetes. Well-controlled diabetes is generally compatible with implant treatment. Poorly controlled blood sugar slows healing and raises the risk of failure, so it should be stabilised with your GP first.
  • Smoking. Smoking is the single most modifiable risk factor for implant failure. You will be asked to stop before surgery and to stay off cigarettes through healing.
  • Heavy grinding or clenching. Bruxism overloads implants and fractures crowns. It is manageable with a night guard and a bite designed around it, but it has to be recognised in advance.
  • Untreated decay or infection elsewhere in the mouth. This is cleared before implants are placed.

Implants may not be the right treatment for you

  • If you are having, or have recently had, high-dose radiotherapy to the jaws, or you take or have taken intravenous bisphosphonates or other antiresorptive drugs for cancer. Both raise the risk of poor bone healing, and these cases need a specialist opinion rather than a travel booking.
  • If you have a significant uncontrolled medical condition, an immune system suppressed by medication, or a bleeding disorder that has not been assessed.
  • If you are pregnant. Elective surgery and CT imaging are normally deferred.
  • If you cannot commit to the follow-up. Implants are not a fit-and-forget restoration, and treatment that stops halfway is worse than not starting.
  • If a single missing tooth sits between two heavily filled neighbours that already need crowning, a conventional bridge can occasionally be the more sensible choice. An honest assessment should tell you that.

Send your recent X-ray or CT scan before you travel wherever possible. Most of the questions above can be answered from imaging and a medical history, which avoids arriving in Istanbul to be told the plan has changed.

The implant fixture itself is the part you will never see and the part that decides how the treatment behaves in ten years. Brand tiers differ mainly in surface technology, the depth of published long-term data, and how easy the parts are to source anywhere in the world.

These are estimates and overlap in practice. A more expensive fixture does not rescue a poor plan, and a value system placed accurately into good bone by an experienced surgeon outperforms a premium fixture placed badly.
